1098-T tuition statement
How do you get your 1098-T? Form 1098-T is a tuition statement (unlike the W-2, which is an income statement) that reports the amount paid toward qualified tuition and related expenses during a given calendar year. If you are a US resident for tax purposes and you have qualified charges and/or gift aid for the corresponding tax year, you may access your 1098-T form online as of January 31. UC Berkeley will send you an email to the email address listed in CalCentral. The email will come from billingnotices@berkeley.edu.
To obtain your 1098-T form, visit the TAB Services website. You will need the following information to login:
- Site ID (11554)
- Username (Student ID)
- Password (initial password is last 4 numbers of your SSN)
- If you do not have an SSN, enter 0000 for the temporary password

Student Health Insurance Plan (SHIP) Waiver
All registered students at the University of California are required to meet the university’s health insurance mandate as set by the UC Regents.
How do I waive the student health insurance? If you have other major medical health insurance that meet the SHIP waiver criteria and do not wish to have dual coverage, you can submit an online waiver application to waive your enrollment in SHIP. If approved, you will see a “Health Insurance Waiver” credit applied to offset the Health Insurance charge on your bill. For more information about waiving SHIP and deadlines, please refer to UHS’s Waiving SHIP website.
If you paid SHIP but your waiver is approved. If you initially paid your SHIP fee and your waiver is ultimately approved, payment that may have been used to cover the SHIP fee can reapply to any remaining outstanding fees, or if all fees have been paid, you may expect a refund within 30 days.
Late waiver fee
A late SHIP Waiver fee will be assessed if your SHIP Waiver application is not submitted on time. Waivers are typically due mid-July and can be submitted for a late fee through mid-August. Visit the Waiving SHIP page on the University Health Services (UHS) website for exact dates and deadlines.
NOTE: A UHS fee on your CalCentral bill is different from a late SHIP late Waiver fee. Refer to CalCentral to view an itemized list of all your fees.
